Κλειστε ραντεβου για εκδοση πιστοποιητικου ΚΕΠΑ απο οφθαλμιατρο ιδιωτη ή δημοσιου φορεα.Προηγειται διαδυκτιακη αιτηση.Εφοσον το καλο σας ματι δεν εχει προβλημα το ποσοστο αναπηριας ειναι μικρο.

Κάνετε αίτηση στο ΚΕΠΑ.
Καταθέτετε χαρτιά από οφθαλμίατρο που αναφέρει την πάθηση.
Συνήθως για βλάβη στο ένα μάτι είναι 32 τα εκατό.
Τα χαρτιά πρέπει να είναι από Νοσοκομείο η Οφθ/τρο του ΕΟΠΥΥ.
